About Us


The FCN/Health Ministries Documentation and Reporting System web-based documentation and reporting system was designed to help networks, nurses, and health ministry leaders of all denominations and faiths meet professional documentation standards. 

The Faith Community Nursing Network will partner with parishes/congregations to document and communicate their efforts to deliver whole person (spirit, body, mind and relationships) health ministries that enhance health and well-being and relieve suffering among congregational members and communities.  
 
The Faith Community Nursing/Health Ministries Network is a community-based, institutionally supported network of Parish Nursing Networks, Health Ministry Networks and independent nurses. It was designed to help its partners to meet professional documentation standards and to increase sharing of challenges, successes and needs in the care of the whole person-spirit, mind, body, and relationships. Networks, nurses, and health ministry leaders of all denominations and faiths are welcome.

Membership in the Faith Community Nursing/Health Ministries Network is accomplished through a subscription to the site and services.

Help develop and lead parish nurse/health ministry projects in which all who subscribe may participate such as identifying parish nurse interventions and outcomes with diabetic clients, or long term care Medicaid/Medicare cost saving interventions of parish nurse/health ministry team. 
   
This site was developed by a grant from the Michigan Department of Community Health given to the St. Joseph's Healthcare Faith Community Nursing Network,  February 2000. The Henry Ford Macomb Hospitals (formerly St. Joseph's Healthcare) Faith Community Nursing Network (FCNN) began developing its documentation tools in 1995 using teams of its parish nurse members. Each year focus teams reviewed the literature and work of other parish nurses and made adjustments to their system to meet their needs and the needs of their clients. The work on the web application site began in June, 2000. In March 2001, pilot testing was completed and documentation using the internet began. The look and function of the site was significantly upgraded March-June 2002. August, 2002, the ability to share the site with others was developed. In July 2003, point of service documentation became available through the use of personal assistive devices (PDAs) and synchronization to the internet through desktops. As the use of PDAs to document using this system dwindled, the use of wireless access has increased.

On July 15, 2007, significant quality improvements to the system were added.  These improvements are the result of collaborative input from FCNs and FCN Coordinators from across the country.

On September 10, 2012, a system rewrite was released. New features and changes are intuitive and easier to use than the previous version. Included changes are option to save your form as draft, an alert when the patient being entered already exists, date validation, age range automatically filling in when date of birth is entered, and no limit on the number of activities or nursing diagnoses/issues you can enter.

On February 3, 2014, the new FCN Professional Process Module section was added.  In collaboration with Advocate Health, the North American Nursing Diagnoses Association (NANDA), Nursing Intervention Classification (NIC) and Nursing Outcomes Classification (NOC) systems was added as an optional section under the title of FCN Professional Process Module to the Individual Interaction form. 


In September 2015, the new Diabetes Prevention Program (DPP) and Other Classes section was added with the support of CDC MI-1422, Health Alliance Plan and Henry Ford Macomb Hospitals funding.  This section supports documentation of an individual's participation and progress in the DPP and/or other chronic disease classes or programs.  Reporting criteria meet the CDC mandated standards as well as other potential population health interests.  Topics and coding can be developed by groups and Networks to facilitate tracking and reporting.


On January 18, 2016, the new Advance Care Planning (ACP) section was added.  HFMH FCN received philanthropic funds to add this section into the system.  This section is heavily influenced by the Gundersen Health Respecting Choices advance care planning system.  The Henry Ford Health System electronic health record uses the same report criteria as those documenting in this system thereby facilitating communication across the healthcare continuum. 


On March 1, 2018, the new “Type of Cost Savings/Avoidance” section and Cost Savings Report was added.  The following network substantially supported this effort: Henry Ford Macomb Hospitals - Faith & Community Health, Sanford Health - Faith Community Nursing & Health Ministry, Memorial Hospital and Health Care Center, Texas Health - Faith Community Nursing & Patient Education, Faith Community Nursing/Health Ministries Northwest and CHI Health - Faith Community Health Network. 


On May 14, 2018, in compliance with new 2018 CDC standards and procedures, additional fields under the “DPP & Other Classes” section were released.  


On May 6, 2019, additional update were made to accommodate data needed for Medicare Diabetes Prevention Program (MDPP) reporting. 


On December 1, 2019, a security system and server upgrade was made to continue to meet standard security requirements.  

 

February 10, 2020, the “Cost Savings/Avoidance section format was greatly enhanced. User can now select time spent, number of visits avoided and/or enter the actual cost of the service provided.  An extensive new Cost Savings/Avoidance report was added. The following Cost Savings/Avoidance Committee members contributed:

On January 10, 2021, the self-service password reset functionality and multi-factor authentication was released.


On March 28, 2021, the new field “COVID-19 Related Activities” was added to all Group Activity categories descriptions to help users to document Covid-19 related community service activities and the following: 

  • New Medication section under the Individual Interaction form 
  • Cumulative Group Activity Report-Notes 
  • Patient Medication Report 
  • Audit Logging Reports


On April 25, 2021, the new patient’s follow up section under the Individual Interaction form and a new report “Individual Interaction Follow-up Report” were added.


On December 18, 2021, enhancements on the DPP and Other Classes section was released with a new CDC DPRP format report added based on 2021 CDC DPRP requirements. The patient profile page has been updated to be able to capture patient’s gender, sex orientation, nationality, language, and English fluency.


On July 30, 2022, the new “COVID-19” section under the Individual Interaction form and two new reports (“COVID-19 Totals” and “Cumulative Individual Activity – COVID-19 Report”) were added. This enhancement is made possible with funding from Henry Ford Health and an Association of American Medical Colleges grant for the Trusted F.A.C.E (Faith based Activation for Covid Elimination) program which is led by Dr. Denise White Perkins and Janet Banks-McElrath. 


On September 30, 2023, the new “Social Determinants of Health Screen” and “Upload Document” sections under the Individual Interaction form were added. These enhancements are generously provided by Common Spirit Health.
